Find Your Motivation!

Weight Loss Motivation Worksheet

We are over half way through the STAR Wellness Pilot Program. Some of you may be feeling discouraged that you have not made as much progress as you would have liked. Some of you may be feeling bogged down with all the work – physical and mental - that goes into changing your lifestyle. This worksheet may help you re-evaluate your progress and give you the extra spark you need to complete the program!

For each of the 4 categories below, choose one or more possible motivators that apply to you.
